Registration Player

On Line Player Registration
1. Team has to be in the computer with a team code to enter team information.
2. Returning player must have a completed, signed registration form, which must be retained by the club or team.
3. After completing the player information in the computer a picture has to be uploaded (640 x 480,email size) hit enter pass completed.
4. The pass will be produced at the NISL office for collection or delivery per your request.

New Player or Transferring Player Online.
1. Team has to be in the computer with a team code to enter team information
2. Returning player must have a completed, signed registration form which must be retained by the club or team.
3. After completing the player information (last two digits on the player card should not be filled in) up load picture.Proof of birth date plus a
copy of the player registration form (any transfer information you may have) must be given to the NISL either by mail, or fax.
4. The NISL will give authorization to issue the pass.
5. The pass will be produced at the NISL office for collection or delivery per your request.

Manual Player Registration.
Player Passes for Previously registered players with your club/team.
1. Teams registration information and fees must be in the League Office prior to or at the same time as the player information.
2. Players registration form for each of the players on your team, completely filled out, plus old player card, picture
(640 x 480,email size) – attached to the sheet)
3. NISL to generate the passes.

New or Transferred Players Manually.
1. Players registration form must be completely filled out, the League office will generate the last two digits of the pass #.
2. Each new players registration must have birth identification, and picture paper clipped to the form.
3. Transferred player with another clubs pass, must have birth identification or the old teams player card.
4. If adding a transferred player which is already on the team for this season 2005/06 then an IYSA transfer form must be sent by the team to which the player is currently registered.
5. The pass will be produced at the NISL office for collection or delivery per your request.

PLEASE NOTE THAT PICTURES CAN BE TAKEN AT THE LEAGUE OFFICE IF THE PLAYER IS PRESENT AND SCANNED DIRECTLY TO THE PASS AT THE OFFICE.

How To Get A Team Code Number
Space # 1,2 = Club Code # (Call the NISL League Office if you need one)
Space # 3 = M - Male / F - Female
Space # 4,5 = Age Group (ie. 08, 09, 10, 11 etc.)
Space # 6 = 1st team, 2nd team, 3rd team, etc.

How To Get A Player Code Number
Space # 7 , 8 = Players last 2 digits of the Birth Year
Space # 9, 10 = Players Birth Month
Space # 11,12 = Players Birthday
Space # 13,14 = Last 2 #’s off last seasons card - leave blank if it’s a NEW player registration

All NISL Player Passes from last season must be returned to the League Office when registering your team.

NOTE:
IYSA Rosters and Add and Release Forms are only available from the League
Office and cannot be downloaded from the website.WHEN IN DOUBT CALL THE LEAGUE OFFICE
