All information requested on the NISL Forms must filled
out in full to be considered a complete application and mailed
in to the league office by the registration deadline.
Step 1
1. Complete the Club/Community Application Form
2. Complete the Team Registration Form for each team registering with the NISL
/ with map of field location
3. Complete the NISL Seeding Tournament Application Form for each team registering
with the NISL
4. Complete the Club/Community Accounting Form
5. Download and Complete the IYSA Organization Statement of Understanding
Register ONLINE or MAIL in all above forms filled out completely along with the
total ($) fees into the League Office by July 1, 2005
Step 2
1. Request from the NISL League Office the IYSA Team Roster Form
2. Download or Request from the NISL League Office the NISL Player and Coaches
Registration Form
3. Complete and keep in the Club/Organizations possession the completed Player
Registration form for each player
4. Complete and keep in the coaches possession the IYSA Medical Wavier
5. If you want the completed player passes mailed back, Please send a self
addressed stamped envelope for $3.00
IYSA Team Registration Form must be completed before player
passes are returned into the League Office by July 1, 2005
How To Get A Team Code Number
Space # 1,2 = Club Code # (Call the NISL League Office if you need one)
Space # 3 = M - Male / F - Female
Space # 4,5 = Age Group (ie. 08, 09, 10, 11 etc.)
Space # 6 = 1st team, 2nd team, 3rd team, etc.
